With any organization there are a lot of forms. The intent is to keep things as organized as possible. Below are Adobe Acrobat (pdf) copies of forms available from Maple Valley Pony. If you are looking for one that is not here, please email us at contact@maplevalleypony.org.
• Background check form – If you are applying to be a coach, board member or an umpire, you must first complete and pass a background check. Please complete the form and return it directly to the Olympia Address on the form.
• 2009_PONY_Coaching_Application – If you are wanting to be a head coach or an assistant coach, you must first complete and submit a coaching application.
• MVPBA Code of Conduct_Coach_– All coaches must sign a code of conduct form to insure that our league remains for the kids.
• MVPBA Code of Conduct_Parent_ – All parents must sign a code of conduct form to insure our league remains for the kids.
• MVPBA Code of Conduct_Player_ – All players must sign a code of conduct form to insure that sportsmanship is held high.
• 2009_MVPBA_Medical Release– All players must sign a medical release form before they can play ball.
• 2009 Return To Draft Request_ – Complete this form if you want your child not to be on the same team as the prior year but instead want them placed in the draft.
• RCS Liability Waiver – To participate and use the facilities at Rock Creek Sports, this release form must be completed and be on file..
